If you are using the lotion twice per week at least 6 months.

I guess so because when my scalp itches, I apply it to my scalp and roots and I think you can leave it in.

Do you still need to use shampoo as well as this

Yes, this is great to treat the problem but you still need shampoo to clean it out of your hair as it leaves a residue.
