Yes but once you’re head has moulded to it it’s very comfortable.

What pillow case will fit this pillow?

Tempur Uk don’t make a pillowcase to fit this pillow, I have found pillowcases which would fit on ebay

How do I know which size is best for me?

I had the same problem, and eventually contacted Tempur. They just said "medium" without checking any measurements, which wasn't very helpful. I guess they might count most adults as medium; perhaps you'd need to be a real heavyweight to need large, and a child to need small?

For neck pain, do you sleep on the thinner side or the higher side?

Higher side for better neck support.
