Is this for physical version or digital version?

If it doesn't say -DIGITAL- you always get a disk. Plus, Sony no longer allows third parties to sell digital downloads. That being said: the disk does not contain the game. The file size is too large(extremely) and has to be downloaded when you install the game. In addition, the disk always has to be inserted to play. I'm not referring to Warzone 2.0 which can be downloaded separately for free.

They don't have the codes. I got emailed from marketing and promotions and they told me to wait 3 to 5 days until the manufacturer replies to them.

Didn't they already make a call of duty modern warfare 2? Is it a remake to the 2009 version??

This is brand new story that takes places after Modern Warfare (2019)|Warzone game. But there are Modern Warfare 1 with online multiplayer and campaigns modes, and Modern Warfare 2 with only campaign mode remastered games.
