How much water does this need ?

2 inches minimum 3 to 4 inches will be optimum. Just need to keep pump inlet submerged. On windy days probably don't run the pump in small bird baths. The wind will blow water away as spout blows water out. It works well on non windy days.

does it have a auto shut off if the water gets too low?

No, it does not. The only ones I've seen with an auto shut off are solar powered and have a very small motor. This is the type that I use for my birdbath. This one is used for a fountain with a deep bowl.

Is it suitable for use in a fish tank?

Yes,it could be used for small fish tank,not fit for big fish tank.

Can you get an extension for the 16 ft cord

I used extension cords on all my 3 bird baths. Remember best to use on non windy days. On windy days water blows away below the pump inlet. This will prevent small pump pump from working efficiently and could burn out pump if not enough water is present.
