Is this 120 or 220 volt?

IDK. It plugs into a regular US outlet.

What is the average run time with one refill? 10 minutes (more/less?)

A few minutes😕 Water chamber is not very large

Does it use tap water?

Yes it does use tap water. I actually used tap water to test it out and it worked perfect for what I needed to use it for.

Do you have to push the steam button the entire time or is it it on/off?

You do have to keep the button pressed. I have some arthritic fingers, but find it super easy, to keep the button pressed while steaming.
