In flat position the height from floor top of seat pad is 18".

how wide is the back pad?

Not that wide, depends on your size. Good enough for bench press, incline, not for decline.

When the bench is in the flat position, how high from the floor is the back of the bench? (the distance from the floor to the support on the back)

From the floor to the top of the pad is 18". From the floor to the top of the structural support for the pad is 15 1/4". The bench is incredibly sturdy and makes me feel safe when lifting heavy. I have read other reviews that had me curious about its construction. I learned that the people who had bad experience were not able to follow instructions on assembling the bench and didn't double check the bolts. When doing decline I do slide off the bench but it's still worth it for the safety for flat and incline.
