Yes I sleep with the highest side as it fits on my neck better. If the small side is better for you that's OK

Is the whole thing made from bamboo?

The cover appears to be bamboo , the pillow interior is memory foam.

Can I purchase an outer cover on its own?

I just use a standard pillow case. It provides enough protection for the pillow

Is it durable? Will it be flatten like the down ones?

Yes it longer than other pillows, it doesnt flatten as it always go back to it's original shape, I bought it as my osteopath had one and loved it
