does this fit a FARO autoclave ?

It did not fit our Midmark M11, so I'm not sure if it fits anything.

Does it include all three pieces, the gasket ring, metal ring and half moon dam?

This kit does not include the metal ring, just the door & dam gasket. However, if your old gasket has the ring you can re-ruse it.
