What is the length measurement from waist to finished in a size 38 waist

If you mean what I think you mean, the depth, for example from waist to the bottom seam around your hips, I just measured an older pair of my 38's, and it's 7.5 inches, or approx. 200mm. With my recent purchase I bought larger, but obviously still have older pairs.

how is waist size determined - is it expanded or otherwise - My waist is 46", yet I wear medium size with comfort?

My husband is a 46in and he found the large were fine. Not baggy as they would have been if he had gone up a size. The company themselves were very good with returns as we mistakenly bought the XL thinking they were right, but really needed the large. Good service and communication. Quality of items is good and they wash very well.
