Is this tool compatible with bosch 18v batteries?

No it’s the 12V (or 10v for the European line), but not 18v. Also be advised it’s not a true StarLock. It uses StarLock blades, but lacks the quick release.

Any blade is in the package?

Does this come in an insert tray? What is the packaging?

No insert tray. Packaging is just the normal consumer packaging. The small plastic box that comes with it is large enough to hold a dozen or so blades or a couple dozen sanding pads.

Is there a starlock adapter to use flat blades?
