I don’t think Amazon has the copper ones but I did see them online when I was searching offset adapters. Just search rain chain offset adapter copper and you will find them.

Mine came with a larger and a smaller plate with the large hole in the center. Does one go inside the gutter and the other outside the gutter?

Yes. The two plates give you ability to cover up the opening you had to make in order to put in your spout. Pop rivets work best for securing the plates and stabilizing the spout. A good silicon adhesive between the plates will prevent water leakage.
