We tested it today and scanner was a little picky on how we held the scanner to get it to read. suggestions on what we can do differently?

I found that the best is to keep it 2-3 inches away from the code. Distance also depends on the size of the barcode

Does this scanner work with any type of 25 bar code. The instruction manual has an option to scan... but the scanner does not acknowledge (beep)

is this scanner plug and play?

Yes it is. Plugged it into my Windows 10 Desktop with no problem.

Why do I have to unplug and replug the usb everytime for it to work?

You must gave something defective on your usb driver. I would suggest getting a replacement. We have never had to unplug in the USB driver even once. This is the best bar code reader we have ever used.
