Is this meant to only be used with a mouse? It looks like it onky comes with 1, so does this mean it isn't for use while typing

For me personally it is meant to be used only for a mouse because it does elevate your arm at different levels, and it will be uncomfortable to type with it. I do leave my hand for quick typing in the arm rest and just move it to the the position needed since it has the flexibility to be move and I am able to type, but it only comes with one and it’s not meant for both hands at the same time.

Is the price just for one or does it come in a pair?

It is just one. It is very well made and adjusts up and down.

where is this manufactured?

I believe the company is in Korea, but where exactly was manufactured it didn't say.
