Would there be a problem using this switch for 120vac at less than 5 amps?

This switch is for 12 volt DC current. Not 120v AC.

Do they make a universal 12 volt rheostat switch for a blower motor to control the speed?

Probably, the problem is each blower motor will be different - amperage could be a problem. You would need to know what the volt/amps is for the existing and then look for specs on others.

which terminal is ground

This is an on-off switch so there is no ground. In a 12v system, auto, boat, rv, you switch the "hot" wire and leave the ground directly connected to the load (light, pump, etc.). I am using this in an RV on the battery box. These days, your smoke detector and/or CO2 detector are always on, so this allows me to turn off the battery when I want to remove it without the annoying spark.
