It comes with the speaker and the bracket.

How does one attach the rubber piece to the inside of bike mount?

This looks like "shock mount" or "shock insert". I think it's not mounted, but inserted behind the speaker once the bracket is installed. It's likely that it's for serious riders or mountain biking where the extra vibration might be an issue.

Would this be loud enough on a fourwheeler?

I have two of these speakers. I'm happy with both. (One is the 4.0 version and one is the 5.0 version. I'm not sure which is which, but one of these talks when switching the inputs, which I really like, while the other just sounds a tone.) I use them on my two 150cc scooters. Under most circumstances, the volume is fine until I get up to around 45 mph. But, in my normal use around town, I rarely go that fast. As for falling out of the holder, I've hit some really big bumps, but it's never fallen out of the provided holder. As a backup, I zip tied the provided carabiner to my handlebar, so if it did ever fall out of the holder, it would just dangle from the handlebar.

Is the digital horn customizable?

No. It have 2 pre-programmed sounds - beep sound and big truck horn sound. Both are very loud.
