How do you get the top off?  Seems to be stuck? Or is there a trick?

There is no trick. It is a normal screw-off top. Underneath there is a paper seal that you must remove the first time you open it in order to use it.  After you have removed the lid you use it by flipping up the half of the lid that shows a spoon for using the pepper in recipes or flipping up the side that shows holes to shake the pepper on.
