Can I use this on a recliner couch?

It will depend on the width of the back of your recliner. The straps are designed to fit on automobile bucket seats. I would NOT get this whole set as the neck portion of THIS set requires a headrest like on a car seat. There is a listing for only the lumbar part (you'd have to search for it).

No, the thickness can’t be adjusted.
