Can this work for the back of my ankle?

Yes, it will work all the way around. I disagree with the first comment on this thread. It actually stays cold a very long time. I would say a few hours at minimum.

Can you sleep with this?

You should not sleep with ice-cold packs on your muscles; usually needs to be used 10-20 min. It can hinder your body's natural healing processes, if left on too long and keeps the muscle highly contracted.
