i would need a medium for a size 9 to 9.5 o.k. women's, correct?

I'm a women's 8.5 to 9 and have a medium. There's about an inch from my big toe to the end of the boot. So yes, Medium should work for you. Don't forget to get a few of the sock liners, makes a huge difference to the comfort around the shin bone (and they are much easier to get on and off - compared to regular socks).

There is a pump built into the side of the boot on the elite model. You can inflate, deflate and control pressure in the various chambers without any special tools or devices. I would definitely get the elite because you have to adjust the air a lot during the day and having to carry a separate tool would be a nightmare.Just my two cents...

with the short model will my toes stick out past the boot?

Short refers to how far it goes up your leg, not the length of the foot bed. So no, your toes should be entirely enclosed by the boot, as long as you order the correct size. My toes were very well protected after surgery.
