Moen typically doesn't include everything that's needed to install. Does this have all the necessary parts?

You would also need a Drop Ell (A725) to connect to the water supply (it is pictured, but not included). Also, this is an add-on function so you would have to pair it with a mixing valve and trim (to adjust temp, volume, on and off, etc) and a diverter/transfer trim and valve (unless you are going to use this in lieu of a shower head. If it is in addition to a shower head you would need a divert/transfer (ie. T4171/3372).

Is this just hand held piece or does it include the complete bar as pictured

it comes with the bar, the hose and the shower head, it does not come with elbow that comes out of the wall.(another 45.00}

It says vacumm breaker is sold separately, although i do not fully understand its usage, did someone install it without a vacuum breaker(and why)?

Vacuum breakers are not necessary. All a vacuum breaker would do is, if you were to leave the showerhead in a tub full of water and for some reason you left the water valve open and the water supply pressure was to drop to nothing, theoretically, that tub full of water could be siphoned back into your water supply.
