Do the screw heads recess into the pad so it does not get near the floor? I am afraid of scratching my wood floors.

Yes, the screw heads recessed well into the pads. The only time I've had scratching occur is if debris gets caught underneath the pad which very rarely happens and is not the fault of the pad itself. Otherwise these work great! I use them on our kitchen chairs over wood floors and after about 3 years I switched them out for some new ones as they wore down.

how do you put them on?

They attach with screws (included). I drilled pilot holes before attaching. I've been using them with a tile floor for 5 months now and they're still working great. The pads with an adhesive backing didn't last a week.

These sliders are screwed into the bottom of each chair leg. When done properly, only the circular plastic portion of the slider is in contact with the floor. I use them on a vinyl tile floor - without any problem.

What is. The best. Slider for carpet?

That depends on what furniture you are attaching the sliders to. The basic principle is the larger the piece of furniture, the wider a surface area of your slider. Teflon type material works best.
