It depends on your pillow style and your sleeping style. **Q: What is the smallest circumference (around the throat) that the Futuro Adjustable Cervical Collar can be adjusted to?** A: The smallest "internal" circumference that this collar can be adjusted to is ~14 -14 1/2". Any smaller and you won't have enough Velcro showing to secure the collar in place.As I indicated in my Review awhile back, I found this to be a good "fitting collar" to determine which fixed size collar is best for an individual. That said, the adjustable part of the collar is in the front-- a 3" wide portion only, in the chin area. The rest of the collar is NOT adjustable. The adjustable chin segment is 3 1/4" tall without tightening the Velcro strap. When you snug the Velcro, you can mush that segment down to about 2 1/2". At that point, the collar looks like a bowtie. The main part of the collar measures 3 3/4" in height, or call it a "nominal 4" Once you wear it a couple of times, though, it's more like 3 1/2". At the very rear of the collar, where you fasten the Velcro to wear it, the collar is 2 1/2" in height. As I also pointed out in my Review, the packaging (see one of the pix on left of Amazon screen) states that the collar goes from 2.5 - 5.0" That is incorrect, totally. The chin segment goes from 2 1/2 - 3 1/4" and the rest is fixed at 3 3/4", call it 4", but certainly not 5". **Q: Can someone tell me the height of the center underneath the chin without the chin strap being tightened and also the height of the sides? Thanks.** A: Height under chin without being tightened 3-1/4 inches (tightened 2-3/4). Height in front 4-inches. Height in back 3-inches. Height on sides tapers from 4- to 3-1/2 to 3-inches in the back.. **Q: The description says this collar is breathable. Have you found it to be any cooler than others?** A: It is warm, but I find it slightly cooler than some other collars I tried. For me, I believe the adjustable area under the chin allows it to be slightly less restricting and therefore not as smothering. The cover is a lose weave, which may be more breathable than certain other covers. It reminds me of the soft fabric used under a surgical boot.
