Is there supposed to be a ORI number on there?

No, ORI is a Originating Agency Identification. They are assigned to law enforcement agencies. The agency that fingerprints you should put their number on the card.

Which revision is this? The most updated is 5-15-17. Picture shows and description shows 12-10-07.

This is Revision 5-5-17 and NOT CURRENT. The current revision is 11-1-20.

Can these be used for concealed carry applications?

You would need to consult your local license Bureau . They did not use this in NY, had their own. If they just require you to get a FD-258 card. Then yes. I used this one for security clearance back ground check for a government agency.

Can I do this myself, without going to police station?

No, has to be at a police station or Sheriff's office.
