Q: What is the width of the leg frames (at the ground)?

Hello, and thanks for your inquiry! The frame measures 18.75 inches at ground level. Note that the frame tapers down, so this measurement is narrower than the width of the frame and cot at the sleeping surface.

Q: One of the pictures says 320lb capacity but the description says 265lb. Which is it really?

There are two Helinox cots, this is the the “Ultralight” and it recommends 265 lbs or less, the “one” (more expensive) recommends less the 320 lbs.

Q: Can an extra leg be installed and would it allow a heavier person to use it?

If a person weighs more than like 250 it won’t work. If you can figure out how to add the extra leg it might work. I only used it once and liked it. I weigh 240. Good luck

Q: How will this cot do inside a tent? do I need to worry about the floor getting punctured?

No, this cot wouldn't puncture the tent floor. It's made to be used in a tent.
